To have your Academic Dismissal Appeal reviewed, please fill out the form provided below. Upon submission, you will receive a confirmation email. The confirmation, and any additional correspondence, will be sent to your Âé¶¹¹ÙÍø student email address. You may only appeal your dismissal once during your academic career at Âé¶¹¹ÙÍø. Any appeal made after your first attempt will not be accepted.
For your appeal to be considered, you must submit a personal statement and a letter of support from a faculty or staff member at Âé¶¹¹ÙÍø. You are also welcomed to submit any additional supporting documents that you feel may be significant to your appeal. If you appeal does not contain a personal statement and a letter of support, it will not be considered and your appeal will not be accepted.
Your personal statement must contain the information listed below:
- What is your major, and why did you decide on this major? Do you plan to change your major if reinstated to Âé¶¹¹ÙÍø?
- To what do you attribute your past positive academic performance? What went well?
- To what do you attribute your past negative academic performance? What did not go well?
- Why do you feel that your academic performance will improve if reinstated to Âé¶¹¹ÙÍø?
- What steps/actions will you begin to take, or continue to take, to enhance and improve your academic performance if reinstated to Âé¶¹¹ÙÍø?
